Patients paired with us via the centralized Minnesota Department of Health’s Questionnaire will have priority. 

If you are paired with LX Medical, and do not agree with our process, we can transfer your referral to another facility.

$About our Process & Costs: 

Our process helps us confirm patient eligibility for monoclonal antibody treatment.  Important steps are listed below: 

1) If paired with LX Medical then we will schedule you for Telemedicine with an LX Medical provider ($59 charge) then, once completed, they determine whether you are formally eligible for in-home monoclonal antibody infusion.

2) If eligible, and patient would like to move forward, we dispatch one of our providers within 24-48 hours to your home (we do our best to book same day and next day appointments).  The cost of an in-home monoclonal antibody infusion is $350, which includes travel, administration of fluids, basic assessment, and monitoring during infusion.  The total cost is $409 – the in-home infusion plus telemedicine.

3) For anyone outside of a 20 mile radius from Minneapolis City Center, additional travel fees will be assessed ($50 or greater depending on your location).  Also, depending on staff availability, if we dispatch someone on a weekend, there is a weekend fee of $100.

4) Additionally, if alternate medical services are decided on between the provider and the patient, the appropriate charges will be assessed for services provided (i.e., advanced medical provider visit: $160-$500.  Wellness/Vitamin infusion: $169 – $248, IV medication administration like anti-nausea medicine: $50, etc.). If you cancel the dispatch of our provider to your home, a cancellation fee may apply (up to $100).

  • We are an out-of-pocket medical practice (we take credit cards, HSA/FSA accounts, cash, or checks).  We do NOT have any in-network relationships with any insurance companies.  We are viewed as an “out-of-network” urgent care/medical clinic, and we can provide you with an insurance reimbursement form to be applied to your out-of-network policy.
  • HSA/FSA type benefits can be applied to any of our services.
  • Once a patient has paid, we can send the insurance reimbursement form to you so that you can work with your insurance company to request reimbursement.
  • The MN Department of Health has informed us that most private/federal insurances in Minnesota will reimburse, or partially reimburse these visits. 



We respectfully request that patients commit to wearing a mask the entire time while our provider is in the home administering treatment, if this is not possible, you will need to seek an alternative treatment option.
